The Senior Vision Coalition is conducting a Community Health Needs Assessment for older adults in the Dallas/Fort Worth metroplex. We would appreciate your participation in this assessment through the completion of a short survey.
The American population is aging. By 2050, 88 million Americans will be 65 or older, compared to the 48 million in 2020. By 2030, one in five Texans will be 65 or older. As the numbers of older Americans increases, the number of individuals suffering from or at risk for serious vision loss also increases. This 10-15 minute survey will assist us in identifying the most pressing health concerns and service gaps in our area.
The survey link below is administered through Google Forms. Your responses will be analyzed as part of our community health needs assessment. The assessment results will be shared with the community through this website. We look forward to developing actionable plans that address the identified needs outlined in the assessment.
